A.T.U 583 Children’s Information Form

With the holidays fast approaching, it is once again time for us to remind you to make sure your child is registered for A.T.U 583 events, including our Children’s Christmas Party!

While COVID 19 is affecting our ability to gather, we are still working out the logistics of giving out the much anticipated gifts to Member’s children. Information on that will be coming soon!

In the meantime, if you have NOT registered your child, please do so with the form below. If you have registered your child in the past, it is not necessary to do so again.

Please be sure you are filling out the form to completion. Children aged 12 and under qualify for a gift.

If you would prefer a hard copy form to submit you can call the office and we can mail one to you or you can pick one up at the office.
